Health Central allows Hospitals to have a powerful digital tool that allows them to obtain real-time information on bed availability, as well as filtering based on the type of available licenses and address, etc. Once they get report back they will be able to send Alf's referral to multiple facility at once and showing Pt clinical and finantial information that makes it easier to find placement
Health Central allows Hospitals to have a powerful digital tool to obtain real-time information on bed availability, as well as filtering based on services provided (Vent, Trach, Ltc, Bariatric), available beds and type of bed available. In turn, the Social Services departments of the Snf can use the system as a tool for the discharge process when needed for Als's search and referrals.
Through Health Central, Nurses(Rn or Social Workers) can begin to fill out the transfer forms (3008) digitally from their portal. Once submitted, the doctors will receive it for review and digital signature from the comfort of their phone if they need it. Also both of them are going to be able to edith or to search for them later on if needed.
Health Central is a digital system on the web that allows you to improve the work of the social worker at the hospital level when finding the appropriate DC(Discharge) option for your patient, whether in need of rehabilitation, a home to live in, dialysis, etc.
Likewise, Health Central would allow you to filter facilities according to multiple parameters such as: Address, Services, Contracted Medical Insurance(HMO), Special Licenses.
These concepts would allow you in a matter of seconds to know, for example, which Alf has a Psychiatry license or which a Nursing Home has the capacity to admit patients on a mechanical ventilator or with a tracheostomy.
